These Membership Terms and Conditions govern the provision of membership services by Cosmo Doctors Ltd (“Cosmo Doctors”, “we”, “our”, “us”).
By joining a Cosmo Doctors Membership Plan, you agree to be bound by these Terms and Conditions.

Membership is available to individuals aged 18 years and over.
Family Membership plans may include dependent children under 18 years residing at the same address.
Members must provide accurate personal and medical information when enrolling.
Membership benefits are determined by the plan selected.
Benefits may include:
Membership benefits are non-transferable.
Unused consultations cannot be carried forward into subsequent membership years.
Consultations included within membership plans are allocated per membership year.
Included consultations must be used within the membership period.
Additional consultations beyond the allocated allowance will be charged at the prevailing member rate.
Membership does not include:
Appointments are subject to clinician availability.
Whilst priority access is provided to members, same-day appointments cannot be guaranteed.
Failure to attend without at least 24 hours notice may result in the consultation being deducted from the annual allowance.
Membership fees are payable monthly by Direct Debit or approved payment method.
Fees may be reviewed annually.
Members will receive at least 30 days written notice of any fee changes.
Membership is subject to a minimum term of 12 months.
Following the initial term, membership will continue on a rolling monthly basis unless cancelled.
Members may cancel following completion of the minimum membership period by providing 30 days written notice.
Cosmo Doctors reserves the right to terminate membership immediately in cases of:
Membership does not guarantee treatment outcomes.
Clinical decisions remain solely at the discretion of the treating clinician.
Cosmo Doctors reserves the right to refer patients to alternative providers where clinically appropriate.
Personal data will be processed in accordance with UK GDPR, the Data Protection Act 2018 and our Privacy Policy.

All complaints will be handled in accordance with our Complaints Policy.
Cosmo Doctors reserves the right to amend these Terms and Conditions at any time. Members will be notified of material changes in writing.
These Terms and Conditions shall be governed by the laws of England and Wales.
